Abstract: Prior to the development of sensitive in vitro serum tests, thyroid function was routinely assessed by measuring radioactive iodine uptake (RAIU) in the gland and by thyroid scintigraphy. The serum tests, along with the physical examination, are usually sufficient to determine whether a patient is euthyroid, hyperthyroid, or hypothyroid.

The usefulness of radio-iodine imaging is not completely outmoded, however. It is still appropriate in evaluating hyperthyroidism, thyroid nodules, ectopic and mediastinal masses, differentiated thyroid cancer, and congenital hypothyroidism.1 This article discusses the indications for ordering RAIUs and scans in a primary care setting and provides recommendations as to when referrals to an endocrinologist are appropriate.
