Medicaid is among the nation’s largest purchasers of health services, spending 2 billion in fiscal year (FY) 2011 on health care and long-term services and supports (LTSS) for 70 million enrollees. Most long-term care (LTC) received by people with disabilities and/or other limitations in their capacity for self-care due to a physical, cognitive, or mental disability or condition is provided by informal providers—family and friends—who give care without compensation. Formal LTC services in the United States, however, are financed by a wide variety of public and private sources. The largest public payer of LTC in the country is, by far, the Medicaid program, paying for almost half (48.5%) of all formal LTC services. Medicaid is intended to provide a safety net for those who cannot afford to pay for LTC services. Because Medicaid is administered and partially financed by each state, there is wide variation in eligibility and benefits across the nation. Medicare and other federal and state programs finance an additional quarter (24.8%) of formal LTC services. Every day, we are exposed to natural radiation such as radon and ultraviolet light, and sometimes also to the radiation burden from medical investigations. Moreover, our lifestyle subjects us to increasing amounts of 'modern' radiation caused by VDUs and mobile phones, which add to our overall load. High levels of radiation are known to cause often profound changes in cell chemistry with inevitable clinical effect. At a certain level, a clear dose response may be observed. However, effects caused by low-level radiation are difficult to demonstrate, particularly when the sources are different and numerous. But this does not mean that there is no effect. Considerable research is being conducted into the effect of low level radiation on the body and first results are presented in this issue. Dealing with all forms of radiation on the body, this issue is of great interest for medical or health practitioners as well as for designers and producers of consumer equipment. This Report builds upon our March 2011 “Report to the Congress on Medicaid and CHIP,” which provided the foundation for a better understanding of the Medicaid and CHIP programs.
