In December 1992 Christopher Clunis stabbed Jonathan Zito, a complete stranger, to death on Finsbury Park underground station in an unprovoked attack. Clunis was a diagnosed schizophrenic. As a result an inquiry into Clunis’s treatment and care was established - leading to the Ritchie report.

This is probably the most famous and far reaching of the recent inquiries caught the public attention in its vivid descriptions of the breakdown in care which led to the attack. However both before that report and certainly since its publication, there have been a series of other such inquiries. The Zito Trust, set up after the death of Jonathan Zito, has published "Learning the Lessons" - a comprehensive and complete record of recent inquiries listing over six hundred conclusions and recommendations.
